Our Support Coordinators recognise the value of informal and formal supports.
Our purpose is to assist you holistically to reach your goals while respecting the input from carers and loved ones. We assist participants to make the most of their plans and link them to the appropriate service providers and connections that may be required.port to all equally.

Psychosocial Recovery Coach

Assists people with mental health issues and guide
them in their recovery journey. They will help you remain motivated and focus
on your strengths, goals and resilience. Assistance to choose, book and manage
your support services, know how the mental health system works and can help
you navigate it. Assist with discharge planning and relapse prevention.

Specialist Coordination of Supports

Specialist Coordination of Supports is a higher level of support, focusing on reducing complexities and managing
barriers.
