Service Agreement with Professional Care Supports

Thank you for choosing Professional Care Supports as your Service Provider. This service agreement sets out agreed expectations around how the Service will be delivered by Professional Care Supports to you. It outlines each party’s responsibilities and obligations, and how to resolve any problems should they arise.

Terms of Agreement

and continues until the Service(s) are fully delivered by Professional Care Supports, or until written notice is received by Professional Care Supports should you choose to end the Service.
Access to your National Disability Insurance Agency Plan: By making this Agreement with Professional Care Supports you agree to provide a copy of, or provide access to, a current copy of your NDIS Plan. This may require interaction with the NDIS Portal or representatives.

Services to be provided: Professional Care Supports agrees to provide you with the Services detailed in the Schedule of Supports.

Fees: The services will be provided for a Fee; Fees are determined by the National Disability Insurance Agency (NDIA). Professional Care Supports will either:
-> Invoice you, the service user directly, if you are self-managing your funds, or
-> Invoice NDIA directly, if they are managing your funds, or
->If Professional Care Supports is managing your funds, we will deduct the fees from the available funds and send you a receipt and balance of your funds each time this occurs, or at agreed intervals for plan management.

The price of Services is outlined in your NDIS Plan. A list of hours claimed against the Plan by Professional Care Supports is available at your request.

Professional Care Supports responsibilities: By enter this Service Agreement, Professional Care Supports agrees to:
-> Consult you to agree how the Services will be provided to meet your needs and timeframes
-> Protect your privacy and confidentiality at all times
-> Treat you with courtesy and respect
-> Listen to your feedback and resolve problems quickly
-> Keep clear records on Services provided to you
-> Explain the Service to you and inform you of any known risks associated with any Services provided
-> Provide you with staff that are qualified and matched to your needs
-> Provide you with staff that have cleared a Policy check and Working with Children check conducted by the appropriate authorities
-> Respect the decision that you make

Participant’s responsibilities: By entering this Service Agreement, you agree to:
-> Work with Professional Care Supports to ensure that the Services meet your needs
-> Treat Professional Care Supports representatives with courtesy and respect
-> Talk to Professional Care Supports if you have any concerns about the Services being provided
-> Provide Professional Care Supports with four weeks’ notice (outlined below) should you wish to end this Agreement.
-> Advise Professional Care Supports with reasonable notice where unable to attend the Services as planned.

Feedback, Complaints and Disputes: Feedback is an important part of the way we review and asses ourselves and our services, and we take it very seriously. There are several ways you can provide feedback. You can talk to any member of Professional Care Supports staff, or you can provide it in writing and email it to machele@professionalcaresupports.com Complaints/disputes: If you wish to make a complaint or have a dispute, you can:

-> Talk to the relevant staff member directly, or
-> Talk to their manager directly (ask for details), or
-> Complete a Complaint Form to be addressed by one of the managing Directors.

Quality Third Party Auditors: Your records may be viewed by a third party auditor to ensure that Professional Care Supports is complying with all funding standards, legislation, and Acts pertaining to disability services. I do not want my records to be viewed by a third party Ending this Service Agreement: Should either party want this agreement to end, both parties agree to give four weeks written notice, unless there is a serious breach of the Agreement, in which case the requirement of notice will be waived. Cancellation and ‘No-shows’: Professional Care Supports reserves the right to claim against your Plan for cancellations and ‘no shows’ as outlined in NDIS pricing rules. To avoid cancellation fees, Clients must provide a minimum of 24 hours’ notice (Mondays to Fridays), or 48 hours’ notice (weekends and public holidays) of a cancellation. In the case of Therapeutic Services, failure to attend an appointment may result in the applicable Service fees being charged to the Plan, as provided within NDIS Funding Guidelines.
